Pharmacist interventions
When a pharmacist resolves a drug-therapy problem they log an intervention against the patient’s visit, prescription or dispense: the DTP category and standardised PCNE problem code, the intervention type and severity, whether it was significant, the prescriber’s acceptance and the clinical outcome. Records flow Draft → Open → Closed.
Clinical & cost value
Each intervention can carry a cost saving and a clinical-significance rating, turning pharmacy activity into reportable quality and value data.
